Lipodissolve vs Liposuction
I serve as an expert plastic surgery adviser for MedHelp.com web site. Often questions on the site relate to issues patients are experiencing after surgery and want a second opinion for reassurance. One of the most common requests involve problems experience after lipodissolve treatments. This procedure involves injection of a chemical or chemicals (most often phosphatidyl choline) that has been shown to reduce subcutaneous fat. The injected substances are often proprietary and secret. They are not FDA approved and are being used off-label. The procedure requires multiple injections over time. They are heavily promoted. The problem is that the amount of fat reduced is not predictable and variable from patient to patient. Patients do experience significant pain and discomfort and lumps after the procedure. Yesterday, the New York Times report a multi-center clinic out of St Louis that is apparently going bankrupt as a result of multiple patient complaints and poor results. Liposuction has been around for many years and in the proper hands is very safe and effective. Buyer beware!

Botox and Myoblock
Recently, a patient of mine was in Los Angeles and was treated with what was described as an injection more powerful than Botox - Myoblock. She called me to ask what I thought.
Botox and Myoblock are two different forms of Botulism toxin (A and B). Both are approved by the FDA, but for different purposes. Botox (type A) is the used and approved for treatment of frown lines whereas Myoblock is used for treatment of a form of neck muscle spasm. Both can be used off label for facial injections to paralyze muscles and eliminate facial wrinkling, but there really is a big difference.
Botox is actually much more powerful than Myoblock and is used in much smaller amounts to treat the same areas. Myoblock is more painful when administered, but its onset of action is shorter than Botox. However, Myoblock typically lasts only about half as long as Botox. Finally, in sufficient doses, Myoblock can also affect the sympathetic nerves resulting in dry eyes and/or dry mouth, a complication that does not occur with Botox.
Based on what she told me, I believe this patient was not given full disclosure about what she was receiving. Interesting.....
